This is so incredibly sad.
And of course I know all these trains have emergency stop switches available to passengers in each car.
Perhaps it happened too quickly for anybody to realize they need to pull the lever or cord or whatever.
Truly the saddest story I've seen in a long time.

Another report:
All those BART trains have operators who sit in little cabs at the front of each train. If the operator saw the incident they could have hit the stop button and prevented the injury/death. However it's not a given that the operator would have been able to see that the woman was attached to a leash/rope. And it's probable that the operator only looked back as the doors were closing, and wasn't looking in that direction as the train started to move. And those trains accelerate rapidly and move fast, as can be seen in the link above.

Well, attach yourself to anything in an area where trains or cars run around is not a good idea. It reminds me of the actress who wore a very long scarf while driving a convertible. The scarf was caught in a wheel and the actress' neck was broken in a second.
